Traditional addiction treatment programs have been shown to be highly effective, but sometimes they don’t go far enough. Since more than 40 to 60 percent of patients relapse after treatment, finding the right care is essential. Individuals with severe substance use disorders may benefit from a treatment pathway known as “residential-plus” treatment, which is a longer program of supervised care emphasizing a holistic approach for patients’ medical, psychiatric and behavioral needs.
“Residential-plus treatment programs are an emerging category of behavioral health and addiction care programs that sit between traditional 30-day residential treatment and long-term, highly supported recovery environments,” Dr. Paolo Mannelli, addiction medicine physician and professor of psychiatry and behavioral sciences at Duke University Medical Center, told Newsweek.
“The term is not standardized, but it generally refers to residential treatment that includes extended clinical, medical, recovery-support and/or transitional services beyond what a conventional 28- to 30-day residential stay provides.”
A typical residential treatment often lasts about a month, but residential treatment facilities are moving toward a longer comprehensive program to provide 24/7 supervised care toward a structured path into outpatient and community-based recovery.
The model is especially suited for the treatment of people who use multiple substances because these people may have more medical, behavioral and social complex presentations, he added.
According to a 2023 study by the U.S. Department of Health and Human Services Substance Abuse and Mental Health Services Administration, roughly half of patients with substance use disorders also have mental health problems. Treating both concurrently helps patients better appreciate the root cause of why they started using in the first place.
“Residential treatment is the foundation of our approach because it brings together psychiatry, counseling, medical care and family support in one healing environment,” Dr. Alta DeRoo, chief medical officer of Hazelden Betty Ford Foundation, told Newsweek.
“This integrated model allows us to deliver truly personalized, whole-person care while addressing the physical, emotional and relational aspects of recovery.”
Newsweek recently partnered with Statista to rank America’s Best Residential Addiction Treatment Centers 2026. Meilin Möllenkamp, head of health care analysis at Statista, told Newsweek, “One of the clearest trends emerging from this year’s ranking is the growing emphasis on personalized and comprehensive addiction care. Many residential treatment centers now offer a broad range of services, providing, on average, 13 of the 16 evaluated treatment options while increasingly tailoring care to individual needs through specialized programs for specific substances or patient groups, such as women and adolescents.”
But residential treatment is only the beginning of the journey toward long-term sobriety.
“Even the most comprehensive program cannot permanently eliminate vulnerability to substance use the way an antibiotic cures an infection,” Mannelli said.
“It is great to have increasing number/better options of care, but no single episode can be healing,” he added.
